Contact Form Joomla Module

This module displays a nice and simple responsive form (for example, contact form) based on bootstrap 4. The details of the form submission are emailed to the recipient. You can specify the recipient name and email in the configuration options.

It supports following types of fields: Text, Textarea, Radio, Dropdown, Email, Phone and Date. You can create as many fields as required by you.
